A 57-year-old woman was killed after being struck by a track-maintenance train near the Ngagel Tirto rail crossing in Surabaya, Indonesia, during the early hours of April 19, 2026.
Video of the incident records the woman on the railroad tracks as the train approaches. Its horn sounds repeatedly before the locomotive reaches her. The footage then records the impact.
Nothing in the video establishes why the woman was on the tracks, whether she heard the horn or whether she intended to remain in the train’s path.
The incident occurred around kilometer marker 67 in the Wonokromo district. Initial accounts placed the collision at approximately 2 a.m., although the emergency report was recorded by Surabaya’s Command Center 112 at 00:1:54 a.m.
A security worker near Wonokromo Station told police that the woman had been walking from the direction of Taman Ngagel Tirto toward the railroad crossing while the train was traveling through the area.
The train was an additional service carrying materials for repairs to the railway line. It was moving from south to north when the woman entered or remained in its path.
The recording captures warning sounds from the train. Residents near the tracks also shouted toward the woman before she was struck, according to information released after the incident. The available evidence does not establish whether she reacted to those warnings.
Emergency personnel reached the location at approximately 00:2:09 a.m. The woman was found beside the active railroad line and was pronounced dead at the scene.
Police said the force of the collision severed her body into two sections. That condition was documented by personnel who conducted the initial examination and recovery operation.
The railway remained active during the response. Emergency crews coordinated with security personnel from Indonesia’s state railway company before approaching the tracks and removing the body.
The response involved municipal emergency personnel, police officers, medical workers and the Inafis forensic identification team from the Surabaya Metropolitan Police. Officers examined the scene while other responders prepared the body for transportation.
The remains were taken by ambulance to the morgue at Dr. Soetomo Regional General Hospital in Surabaya. Police requested both external and internal postmortem examinations to determine the medical cause and manner of death.
The woman carried no identification when authorities examined the scene. The portable identification system used by investigators also failed to immediately establish who she was.
Initial reports therefore listed her as “Mrs X” and estimated that she was approximately 40 years old.
Information published the following day identified the victim as Rukiyati, a 57-year-old resident of the Bratang Gede area of Surabaya. The updated age replaced the preliminary estimate issued while her identity remained unknown.
Police found no evidence of injuries unrelated to the train strike during the initial examination. The statement referred only to the visible injuries caused by the collision and did not represent a final autopsy finding.
Investigators did not announce evidence that another person pushed, attacked or forced Rukiyati onto the tracks. No other person is visible making physical contact with her in the published footage.
Authorities also did not determine publicly whether the death was an accident or the result of a deliberate act. Police said that question remained unresolved after the scene examination.
The video proves that Rukiyati was on the tracks and was struck by the approaching train. It does not establish her state of mind, her reason for entering the railway area or whether a medical or psychological condition affected her actions.
The publicly available information does not indicate that anyone aboard the train was injured. No derailment or significant disruption to the railway infrastructure was reported.
No official autopsy result, final classification of the death or additional finding concerning the circumstances had been published by June 12, 2026.
